Below are links to commonly requested forms and information. If you need other information not currently listed online, please contact Camp Mishawaka directly at 800-308-5011. Adobe Acrobat Reader is required to view these documents.
 

  • Download 2012 Registration Form
    You can fill out this form on a computer and then print it, sign it and fax
    or send it to Mishawaka . You can also print the blank form and fill it in
    by hand. Instructions for mailing and payment are included on the form. If
    you would like to fax the form to us our fax number is 218-326-9228.

  • General Information
    Read for some quick answers to frequently asked questions about the summer camp experience.

 

  • Parent Handbook
    Includes all the information parents need to know. This is the same document that is mailed to you in a booklet form after we receive your registration. Includes information on mailing address, health care, packing list, transportation, as well as lots of other general camp information. This document is 200kb and may take a little while to download, especially using a dial-up connection.

 

  • Packing List
    This will help guide you in the packing process. Remember, camp is an outdoor experience so don’t pack things that can’t get dirty.

 

  • Pre-camp Questionnaire
    This questionnaire gives us an opportunity to learn about our campers before they head off to camp.

 

  • Health Form
    This form needs to be filled out and mailed in prior to the camper’s arrival. Please keep a completed copy for your own records.

 

  • Asthma Form
    If your camper takes special steps to manage asthma, please fill out this form in addition to the health form.

 

  • Charge Authorization Sheet
    This form authorizes specific camper charges and spending.

 

  • Trip Permission Form
    This form gives permission for your camper to attend various extended trips.

 

  • Horseback Riding Permission Form
    This form gives permission for your camper to participate in our horseback riding program.
