Read for some quick answers to frequently asked questions about the summer camp experience.
- Parent Handbook – PDF
Includes all the information parents need to know. This is the same document that is mailed to you in a booklet form after we receive your registration. Includes information on mailing address, health care, packing list, transportation, as well as lots of other general camp information. This document is 2MB and may take a little while to download, especially using a dial-up connection.
- Packing List – PDF
This will help guide you in the packing process. Remember, camp is an outdoor experience so don’t pack things that can’t get dirty.
The following forms need to be filled out before the camper arrives at Mishawaka. You can fill out some of the forms online as indicated. Some must be printed and mailed or faxed to camp.

This questionnaire gives us an opportunity to learn about our campers before they head off to camp.

This form gives permission for your camper to attend various extended trips.

This form gives permission for your camper to participate in our horseback riding program.
- Spending Account Calculator – PDF

This form helps you determine how much money to deposit into your camper’s spending account. It will help you add up any extra charges including those for: extended trips, horseback riding, camp store purchases, and bus transportation to and from the airport.
- Health Form – PDF
This form needs to be filled out and mailed in prior to the camper’s arrival. Please keep a completed copy for your own records. This form is not available to fill out on-line, and will need to be printed and either mailed or faxed to us using the info at the bottom of the website.
- Asthma Form – PDF
If your camper takes special steps to manage asthma, please fill out this form in addition to the health form. This form is not available to fill out on-line, and will need to be printed and either mailed or faxed to us using the info at the bottom of the website.
